Emergency Series - The Sun Does Not Always Work..

Some people are just stone-cold deaf. Like there is literally nothing you can say to them that will go down well with them, they are the bigots who refuse to listen to any other person’s opinion and think they know too much, so full of themselves.

The irony is that their brains are myopic and limited, they don’t realize things are going to get out of hand until it is much worse, then they come running for help, panicking like they just overdosed on LSD, and hoping for a miracle that is almost impossible.

To be honest, it takes a lot of patience to be a health worker, whether you’re a nurse like me, a doctor or a pharmacist, because a lot of things patients do are baffling, sometimes they get you raging mad and make you feel like giving them a punch in the throat.

They are hard to believe and make you begin to question humanity sometimes. That’s just what this story is about.

A baby was rushed to the emergency room, not even a baby, a neonate, because he was less than 4 weeks old. I have never in my career as a nurse seen a baby’s eyes that yellow before. The baby’s skin looked wrinkled and looked like he had lost some weight. He didn’t even have the energy to cry properly and was pale-looking. The parents brought him, with the father’s mother and sister tagging along.

They had requested to be discharged against medical advice to go and perform ‘family rites’ on the baby as they claimed it was their custom to do so. They were supposed to wait for phototherapy as the baby’s blood bilirubin level was high but they only waited for two days after delivery before deciding to go home.

The mother said she was putting him in the sun at home for ‘fresh light’ to reduce the severity of jaundice and when she noticed it was not working and that the boy was looking pale and yellow that she decided to visit the hospital. She said she was surprised as to how the boy deteriorated fast.

The boy’s blood was taken for bilirubin test and it came out to be terribly high, 36mg/dl. It was the highest I have ever heard of or seen before. The baby’s blood level was also low and he had high WBC, about 18,000. The baby was in a bad shape and even had to be placed on oxygen. He was also placed on antibiotics. Phototherapy couldn’t help the child anymore at that point because the bilirubin was waaay too high and had to get his blood totally removed and exchanged for new donor blood in a procedure we call, exchange blood transfusion, EBT.

The parents were asked to sign a consent form. The boy’s parents would have to spend way much more money, only if they had been patient enough to wait for phototherapy at birth. The baby’s weight was even a little over 4kg at birth, which could increase the level of jaundice as well. While we got the blood ready, a fight started between the mother of the baby and the father’s sister, the father’s sister called the mother something in a language I don’t understand, then the child’s mother pushed her sister in-law hard till she fell down and hit her knee on an iron door close by.

The child’s mother held her cloth and we had to call the security to take them out. The father of the baby just ignored them completely. Later on the mother came back alone. I didn’t know what led to that.

Anyway, we got blood ready and a pediatrician was called to perform the procedure, it involved passing tubes in and out of the baby, one tube transfuses new donor blood while another removes the baby’s toxic blood. It’s a procedure done with extreme care and very slowly, as if it is done fast could cause the baby to be hypoxic or cause abnormal cardiac changes which aren’t good. That’s why a pediatrician was called to do it. It was successfully done.

During the procedure, the parent were told to wait outside and were let in after. The baby was then taken to the neonatal unit for further care. I hope the baby has not had any complications yet because high bilirubin can cause brain damage which would slow down the rate of development and learning.

The complications were explained to the parents. The mother was looking so sad and exhausted. I don’t know where they got the idea that sunlight works for jaundice, even if it does, there is a level of jaundice that sunlight cannot effectively cure and the child will have to be on phototherapy. If you are currently pregnant or you just had a baby, it is important you listen to the doctor's advice as regards jaundice checks and phototherapy, don't be a hurry to go home for naming party. Its only a baby that's alive that can be named.

    1. Infant jaundice usually occurs because a baby's liver isn't mature enough to get rid of bilirubin, the product of destruction of red blood cells, in the blood.

    2. This was what made Wilson's son to die. They refused to take him to the hospital becuse their faith is against talking drugs.
Thank you Nurse. I always enjoy reading your article.

  9. Sad! I hope the baby gets better without complications.
This kind of scenario just played out in my place of work but not this bad, my colleague claims urine cures conjunctivitis (Apollo) and she has been using urine in her eye since yesterday cos she started feeling the symptoms but her eye is better now due to the urine usage. She was justifying it and even saying she'll continue with it. To say i was disappointed is putting it lightly.

  10. Nurse no vex o...una no dey fit shout for this people? This all boils down to husband's family interference in their sons marriages. The baby's mother probably wanted to do the right thing but her mother and sister in-law put her down and decided to do their nonsense family tradition. This scenario plays out in a typical Nigerian home. The wives don't always have a say when it comes to their own babies and the sad thing is the husband never supports his wife. Personally, I think women should always use Google and talk to their doctors. I mean, mama's way of taking care of a baby 30 years ago can't be applied in 2018 na but they will swear that it works. Those days their children faced several illness and even death but they will never say in stead they will blame it on abiki spirit. Mothers, please be wise! Your doctor is the only person you should trust and also trust your instincts.

  11. The sunlight works but not at a very high rate of bilirubin.my son had jaundice, was on phototherapy for about 5days but since the test showed the bilirubin was going down,we were discharged and told to use d early morning sun but to come back after 2days for test and d level was then ok.

    4. Na wah oooo, your MIL just made me laff. The funniest thing is that most of us women go through some of this nonsense with our MILs. If I start my story and the experiences with my MIL we no go comot here today. Each time I remember some of the things she did to me with me going through CS pain at those times, I always pray fervently that God will help me bless her daughter with exactly a MIL like her mum in JESUS mighty name 🙏 and so shall it be. Amen

  15. Minnie rexpect, that urine treatment works with speed. I use when I detected I have Apollo.

My aunt's step daughter gave birth to a baby with jaundice, she was told to wait for the baby to be treated, she refused and told them "I reject it in Jesus name". They did not know when she left with the baby. The Jaundice worsened, the baby was taken back to the hospital for treatment with a huge amount of of money spent.

  16. Hnmmm, I don't know why women joke with jaundice, u will be hearing advice from different corners, use pawpaw water, early morning sun will cure it etccc. Please mothers, let's listen to our doctors, staying back at the hospital for extra 5days or less, if your baby has jaundice is not a crime. Please!

  17. There are different types of jaundice, breast milk jaundice which develops few days after birth in breast milk fed babies etc. Knowing which type helps you determine the treatment. Sunlight works for low level of bilirubin. For breast milk jaundice, switching to baby formula helps

    1. I was doing exclusive breast feeding with my son, when the health visitor came home to check on him 2days after we were discharged she told me to go straight to the hospital that my son had jaundice. Luckily for us he didn't need to be put under light. When I was told he had low bilirubin and formula had the required content for him, you need to see the way I asked if I could get formula for him because they usually give out formula if u want on maternity ward. That's how I stopped my exclusive and started mixing oooo. Abeg, after suffering morning sickness that entered 7 months and CS pain, why kill myself on exclusive breastfeeding 😏😏😏

  18. That was how my friends mum inlaw put hot Robb inside her babies anus because he didn't poo for 2days. I can't take such. Thank God for my mum. She respects our opinion. She doesn't do outside what the doctors says.

I pray the baby gets better. Please give us feed back

That was how one mama I don't know o! Left her daughter's bedside to come and tell me not to put my daughter under the light again. That I should go home and put her under the sun, and give her pawpaw water to drink. Imagine!
She also said I was the cause of the jaundice, that my dress is yellow, and I also use yellow shawl to cover the baby. Hian !
I just said thank you and face my work.
