EKO Hospital Press Release On The Tragic Loss Of Mrs. Dolapo Adesanya

RE: PUBLIC INTEREST IN MRS. DOLAPO ADESANYA
FOR IMMEDIATE RELEASE

Maryland, Lagos April 11, 2015 –

The management’s attention has been drawn to a one-sided social media narration of the unfortunate and tragic loss of 36 years old Mrs. Dolapo Adesanya after child birth while under the care of EKO Hospital on Sunday, April 05, 2015.

In addressing the misinformation contained in an expectedly emotion laden though vitriolic missive by a “good samaritan” over what is truly a sad and tragic outcome of an event, the hospital will consistently respect the ethical patient-doctor privileges even as it acknowledges the deep sense of loss being felt by the family, friends, colleagues and indeed her husband and parents.

Given the public interest in the matter however, it is important to be aware of these facts:
  • Every loss of life is regrettable and death is occasionally inevitable in a hospital, EKO hospital will continue to dedicate itself to the highest level of healthcare service which allows it to treat from the simplest to some of the most complicated cases for almost forty (40) years. It also uses all in its capacity to avoid preventable fatal outcomes.
  • The late Mrs. Adesanya became our patient from October 2011 and delivered one previous pregnancy successfully without any incident in a hospital where thousands of babies have been delivered in the last two years with mortality rate way below half of the National average despite being a tertiary referral center.
  • The late Mrs. Adesanya booked late for antenatal care at 26weeks.
  • The antenatal period was uneventful until she was admitted into the labour ward at 36 weeks gestation complaining of backache and lower abdominal pain resulting in her difficulty in walking. Clinical evaluation was performed. Certain investigations including abdominal ultrasound was performed and she ruptured her water during the latter. She was taken to the labour room where she was delivered of a premature baby.
  • She delivered the current baby prematurely at 36 weeks in a position that she found most comfortable in the labour room and the baby was resuscitated and is now in a stable condition in the special care baby unit. The administration of care and counseling to the late Mrs. Adesanya is well documented. Appropriate modalities of care were discussed with and offered Mrs. Adesanya at every stage.
  • The immediate post delivery period was satisfactory and she was transferred to the post-natal ward in a stable condition. Her condition remained stable for 34 hours after delivery.
  • On the faithful day she suddenly developed a cough, became breathless and collapsed and all efforts to resuscitate her proved unsuccessful. A clinical diagnosis of pulmonary embolism was made. Pulmonary embolism sometimes occurs in people with predisposing factors e.g. post-surgery and post deliveries etc without warning.
  • Further insight/confirmation into the cause of death and other predisposing factors would have been revealed by post mortem examination which the family refused, and we had to respect.
  • The hospital did not abandon the corpse but gave room to the request by the family members to exercise their faith and belief in the efficacy of prayers in the post mortem period which lasted 24hours; this was also equally respected.
  • The hospital participated in a meeting on Friday, 10th of April 2015 with the HMO and her employers.

The Hospital wants to state categorically that there has been no compromise to patient safety in this case, which is always the top priority. We appreciate that sudden unexpected death in whatever form can be traumatic and can lead to sometimes inappropriate actions from those affected understandably.

The hospital respects and asks that the public allow the family to grieve in this trying time. During this time, the hospital will continue to do its best to minimize the trauma for the family.

We thank patrons and concerned members of the public who called in for the support offered both the family and the hospital.

Our heartfelt condolence goes to the husband and family of Mrs. Adesanya.

      Odiegwu!

They can afford to do this because no one has a video recording, perhaps, of how it all went down.

Now there are two VERY DIFFERENT accounts of what happened.

Eko hospital, I don't believe you.
If she gave birth in a comfortable environment, why would it be alleged that she had the baby in an elevator?
And that she was first attended to, over 24 hrs after she was brought in?
And that it was leg pain that escalated into this whole mess?

      I don't believe eko hospital.

    3. Fool labour pain usually manifests as waist or back pain which radiates to the legs and most women can't walk normally or even at all according to their pain treshold but its ALL NORMAL LABOUR

    1. I almost lost my first son when I started bleeding in my first trimester. Eko hospital stabilized me and today he's a healthy and normal 7yr old. His delivery was professionally done. The nurse never left my side all through labour. Monitoring his heart beat and the timings of contraction.

Fast forward to my second child I had in Ikeja Medical hospital upon referral by a colleague. Nurses were rude and no attention was given to me until my water broke while in the general ward. The delivery room hadn't been prepared cos they "didn't know I was that close, despite my contractions". Thank God my child came out healthy too.

To cut the long story short, I will go to Eko hospital anyday over the other. Different strokes for different folks. My experience there was pleasant. Let's not rubbish the hospital. That wouldn't be fair.

So sorry it turned out bad for Mrs Adesanya the second time around. May her soul RIP, Amen

    6. Most Government hospitals don't value lives. The almighty mother n child hospital in Akure showed me pepper. Their nurses are so rude and not accommodating. If u scream during labor contraction, they will shout on you like goat...I had a very bad experience with them. The CMD sent me out of the hospital because I was shouting I needed a CS and it was due to the pains of oxytocin, though my 1ST. The wicked women sent me out. Thank God for a good Samaritan, a retired midwife that has a clinic, she saw us outside the hospital and asked us what happened and we explained to her. That's how she took us to her clinic, very neat place and new, I was treated like a queen. Did I tell u I was 8 cm when I was sent out? I put to bed after 20 mins that I got there. Since then I have made up my mind not to use any Govt hospital again.

    7. Madam you have not said the truth biko. If they sent every woman screaming for surgery out of the labour room then no body will deliver anywhere. And government hospitals send Pts out with REFERRALS unless you signed against medical advice which I can bet my last Nairaland you did.

  27. Thank you for posting this Stella. Immediately I read the petition and saw the clinical circumstances surrounding the death, I knew it was highly likely she had either a massive pulmonary embolism or amniotic fluid embolism...more of the former though. And even in highly equipped centres, she may have still died. I didn't want to comment though cos it will seem as if i was just supporting my colleagues. I knew it was a matter of time before the real truth will surface. That said, all health care providers need to be careful and as professional as possible. Patients need people to blame for mortalities...who better to blame it all on than the health care provider?? So many people on the lookout for one doctor or the other to hang...First do no harm and also protect your own interest. May the soul of the departed rest in perfect peace and may God grant her family the fortitude to bear this irreplaceable loss...amen

What about about the negligence to her pre-op management

A pregnant woman presents to the hospital with leg pain, abdominal pain and back pain and you just give her paracetamol as analgesic and dismiss her home

She came back the second time still complaining!!!!! That's red flag right there, and that should ve hinted the doctor in charge that there was more to it, no Doppler test carried out , jUst abdominal ultrasound, no prophylaxis given.....::::
Pls delay of a consultant who would ve properly picked this signs....

We know pulmonary embolism doesn't just occur , there are co morbid factors.
They failed to pick up the signs early enough, before the emboli went to the lungs and we all know , there isn't any cure for pulmonary embolism

In summary !! The doctors in charge were 'negligent'

      Delete
  28. I was not expecting anything different, they would not agree to anything that they where accused of because it will be bad for their reputation and business. Stella in Germany where you leave do people, especially families who lost a relative in an hospital and accused the hospital of negligence and inhuman acts to the victim, will the family let the matter rest because the hospital respond to the accusations. Am not saying the family of Dolapo should not let things go and grieve in peace, all am saying is that investigation should done and post mortem should be allowed by the family in order to fully ascertain the cause of death

  31. May her soul rest in peace. To Eko Hospital, Nobody is saying that the deceased was not attended to...but the case here is was she treated in a timely manner? I also blame the family of the deceased because it is common knowledge that the treatment you get when on HMO is poor because the hospitals complain that the money is too small. But why do hospitals accept anyway? I was once on Lagoon hospital health management plan and due to my small position at work, the company paid 12,000 for medical services for me per annum and this plan covered a variety of treatments including normal vaginal child delivery. Now fellow SDK visitors, can 12,000 cover child delivery anywhere in the world? Is that not risking someone's life? A staff of one of these banks once went to get treatment in a good hospital in lagos while on HMO plan and after taking the drugs given to her for days, symptoms persisted. She went back to the hospital and a nurse confessed to her that hospitals don't give people on HMO the best drugs. Can you imagine that the patient made a case about it and the bank took the matter up and the hospital denied what the nurse said. Do you know that the said bank still subjected staff to HMO. Meanwhile these employers deduct medical allowance from staff remuneration.

    1. Oh no... what a pity. People should understand the system we are in and make their own plans.
Some ladies at the General Hospital here will say " erm they've got the best gynae and specialists here that why am using only this place" and I will always tell them "just say because its free and you have no money. How many times have the specialists attended to you during ante natal or take delivery? They all have their private clinics." The gynae am under, consults for 3 different hospitals. So the best bet is to register in a good private clinic and still at the GH just for backup.
Imagine going thru all this discomfort for 9 months and one story come follow.

      Delete
    2. You've said it all.
I was using a very good hospital in VI. Only for the HMO to complain that the bill being given to them was too much and as such, they were removing the hospital from their list of HMOs, because the hospital refused to compromise their standard.

      Delete
    3. What you have said is partially true depending on the particular plan you're on with the HMO.if you're on the least plan, the money will be small but if on a higher plan definitely you'll get the best drugs ever.before I started working I was a dependent on my sisters HMO and anytime I go to any hospital I always get the best services and drugs being administered. But when I started working due to my low position in the office I was on a shitty plan with my company's HMO.I never bothered using it cos my sisters own was still active.so I guess your company placed you on the least plan that's why its 12,000 per annum.it is well.

  33. Baseless defense in my opinion. Can the hospital explain how and what they meant by the lady rupturing her water during ultra sound? How did she achieve that?
Can the hospital explain what they mean by the lady giving birth in a position that was suitable to her? The latter has to be one of the dumbest things I have read in 2015. Remind me where the midwives and doctors were when she did that or were they present and allowed her to do that knowing the life threatening consequences of that action on the child. I don't think any mother will know that her actions can put her baby's life at risk and still want to go ahead no matter the pain she was going through. Since she came back post delivery with complaints which you are attributing to post delivery (pulmonary embolism) and you are fast enough to define it, what measures did you put in place to ensure that aspect was taken care of. Why will you allow a dead body to stay unattended to for a whooping 24hours? I don't have a problem with the family desiring to try prayers but leaving that corpse there for not even 12hrs but 24 shows so much complete laxity on the hospital's end. Her booking at 26 weeks doesn't hold water, that was still enough time for you to have identified Any underlying conditions she may have had if at all you did your job well. Am sorry there are way too many loopholes in this story and whoever coined this write up should can do better.
Note: I am not related to the deceased. This story doesn't just add up at all... sorry.

  35. In clinical practice,a whole lot happens 'behind the scene' that you as a doctor can't say out as some patients shield underlying ailments even from family members and friends hence the whole doctor-patient privilege thing!
I however noticed the significant NEGATIVE contributory stories about this same hospital from BVs that flooded here when this story first came out...the management certainly needs to look into it as they may be unaware of the claimed indifference.
Such a sad one...may her soul continue to RIP and may God grant her family the fortitude to bear this irreparable loss...Amen!

  36. It's a lie jor, what would they have said? A good hospital will know how safe a pregnant woman is even @ 36weeks if that's when she starts antinatal, that's no excuse. So what is their excuse for others that have come up to say their own experience in the same hospital? God knows best. RIP to the dead.

I am not sure about Nigeria but here they don't take you seriously until you have missed two periods. Before conception start taking folic acid and Once you miss one you just start prenatal vitamins .

    2. MzAnon, I beg to differ.

Staff of govt hospitals can be RUDE, but you will most definitely get proper treatment.

If you complain of stomach ache, you will be made to undergo tests.

At this point, your statement about govt hospitals being free comes under question.

The tests which will be run, are they free?
The drugs which will be recommended, to alleviate pains in the meantime, pending the outcome of the tests, is the patient not going to pay for it?

The services could be free for minors, but not adults.

I'm not trying to say that people have not had terrible experiences in govt hospitals, but still govt hospitals are very much preferred by many.

And their services are paid for.

Every day the bills piling high and them dragging their feet, we transfered her to a "local" hospital where she had her surgery done and she is doing well! very negligent staff! I did meet 2 angels there (an admin staff and the dentist, the rest? Very lackadaisical!). Oh yes and I am a doctor! It's unfortunate Mrs Dolapo died, I smell a rat in this story, Dolapo family please get a lawyer, report to the MDCN let a formal inquiry and investigation be done, let the post mortem be done and if they are guilty of her death. Let them be held accountable! Their unseriousness has to stop sometime!

  47. Emm…. What happened to “respect the ethical patient-doctor privileges”? You went straight in to give us her medical history/condition. I love the way they summarized/skipped the catalogue of blunders and how she was not seen by a consultant for days and declared “he immediate post delivery period was satisfactory”. It is not for you the hospital to declare whether or not it was satisfactory. That is for her husband to do so please hold your fire.
You say “A clinical diagnosis of pulmonary embolism was made”, but I notice this was at the very end when she collapsed. Should alarm bells not have been ringing right from the very start?
All I see in this response is pure damage control. I do not know Dolapo, but I find the arrogance of this hospital unbelievable. That they can write a response which in no way addresses the direct allegations of failings that were made against them. I would advise them to try again and this time address issues like – was it acceptable that she was dismissed the first time she visited the hospital complaining about pain in her legs? Is it the case that there were delays in her being attended to by a consultant? Was it the case that there were delays in her getting a scan? Did her baby’s head start emerging while she was in a wheelchair being wheeled to theatre? Did she indeed collapse after an injection was administered? Address the allegations and stop dribbling around the issues that were raised.
Last of all, you have no business asking the public to leave the family to mourn in peace. If you have nothing to hide, why are you trying to avoid the family being comforted by well meaning individuals?

  48. Please, what did registering for antenatal care at 26 weeks have to do with her death? Are they implying that if she had registered earlier, she would not have died? I am sorry but this response is most un-professional.
I cannot believe the nativity of folks out there or then again maybe the hospital has a team of ghost writers, frantically posting comments in their support. I fail to see what they have said that shows they in a positive light and that they attended to her in an appropriate manner and yet people are acting like they have just given a robust rebuttal to the allegations. I guess I should just blame it on our educational system which has left us with a mainly semi illiterate educated population.

    ReplyDelete
  49. It is sad when hospitals do not take responsibility for their negligence but if you have given birth under HMO and as a private patient you would know the difference. The government needs to set up an agency to control our healthcare system so people have a place to take their complaints to when they feel aggrieved

  52. If she registered for ante-nantal late at 26 weeks that doesn't mean she should be treated badly. When I was pregnant with my third, I registered for my ante-nantal at 30 weeks. This was because I was still seeing my doctor regularly for check up and if I had any complaints. Late registration doesn't mean she wasn't going to the doctor at all. I am sure she was especially as she noticed on time that they was something wrong. Is eko hospital trying to tell me that if an emergency is brought, she wont be well attended to cos of the late or no ante-nantal services? The woman has been using the hospital for 4 years that means they have her case history for 4 years. Late registration is a huge joke to use to put a blame on the dead.

I have actually been to eko hospital and if I try to describe my experience....it was very terrible. The level of non-challancy is horrible.I was born in Lagos and had all my 4 kids in Lagos. I have been to over 30 hospitals but I can say Eko Hospital is the worst. I am happy this is made public and I hope the medical association actually investigate all these allegations. I was deceived by the building but I had to run for my dear life after experiencing their careless service. Even after reporting to their senior staff, nothing was done about it. All my 4 kids were conceived via HMO and I kept changing whenever I got an unpleasant service but none of unpleasant service were even half of my experience at Eko Hospital. My advice to pregnant women and mothers is that make sure you have a documented evidence of any unpleasant service you experience by writing to the HMO and getting an acknowledgment copy then don't hesitate to change ur hospital. Pregnancy is a period you can get impromptu surprises so don't take anything for granted.
This is a lesson for us all. God bless you.

  55. Speaking from a distance so usual caveats apply. If this was an exam question, it is likely she had a DVT which travelled to cause a PE! This is a big fail for the medical team. Pregnant, likely overweight, sedentary (due to traffic alone), pain and swelling in legs is a DVT until proven otherwise. Add that to a traumatic labour. She should have been on thromboprophylaxis!!!!.

    ReplyDelete
  56. Oh no... I don fail exam o. Where did I see leg pain .... Una sorry o. Abeg ignore my early Tory. Abdominal pain don change diagnosis. Pele.

    ReplyDelete
  57. This their response is very phony. If it is true that she checked into the hospital with severe leg pain(as opposed to only severe abdominal pain the hospital mentioned)then it is apparent that trained doctors should have immediately known that she was at risk for pulmonary Embolism resulting from blood clots that would have dislodged to the lungs. They were careless and negligent, because immediately following delivery they should have done evrything to do appropriate blood studies and treatment to prevent the blood clots from getting to her lungs, even if nah Aspirin depending on her blood work. All this their flimsy excuses dont change the fact that they were not thorough in her Management.May she rest in peace.

  60. How can a hospital who should look into allegations labeled against them call such one sided? Lack of incompetence among your staffs should be blamed. I call your press release one sided not the allegations. You can't expect families to move on with such issue when it hasn't happened to your family members.
Eko hospital used to be very good hospital but i am sure their performance have dwindled due to lawsuits within managements (CEO'S) which is affecting the hospital. I took my cousin to the hospital years ago and he was treated very well after robbers blasted his laps with bullets, he lost so much blood but survived brilliant surgery performed on him by Eko hospital doctors.
He was lucky even our registered hospital refused to admit him then.
Do we still have such doctors in same hospital, No of course I doubt so because Nigeria lack continuity/
management and maintenance culture. I believe the management of the hospital should look into the conduct of their doctors and nurses through investigation to change things because some nurses are so lousy while doctors are after working in 2hospitals to keep up with their bills as most do than concentrate in a particular hospital.
I don't think is right for the matter to rest, it should be followed to it's logical conclusion and the management sued. Do expect Eko hospital to accept any wrong doing? No way. Nigeria and it's system in general do not accept any wrong doing be it in private or public capacity.
Most death (70%) in the hands of Doctors are preventable in Nigeria. My family members were victims as i have always said,Misdiagnosis, lack of compassion, training and retraining among them are some of the problems they face. Medical system is rotten like our educational system. Victims husband should have taken her out of the hospital if he felt wife wasn't treated well or attended but i guess as a typical Nigerian he will say "God will deliver her" not knowing God can't help your situation when you do not help yourself or make a move.

  61. EKO HOSPITAL NEEDS TOTAL OVERHAUL , OR BE CLOSED DOWN!!!

    ReplyDelete
  62. Sigh. This is indeed heartbreaking! The hospital should not be releasing statements for the general public. Medical jargon is alien to most. They should be talking to a coroner. Infact there should already be a coroner's report on this case. This is a death suspected to be caused due to negligence and lack of proper work ethics on the part of the medical personnel involved in this case. Hence, a case for the coroner's office and jurisdiction.

As soon as the late woman complained of leg pain from the very beginning, a novice would suspect embolism immediately not to talk of medical professionals. This is really disappointing and heart wrenching. The problem is lack of proper work ethics, not infrastructure or even personnel that know their stuff, it's the ethics that's the issue.

I remember doing clinical rotations not too long ago. This kind of incident is enough to suspend and even revoke med/nursing license.

What I get from this statement is a hospital's bid to protect reputation. The statement actually said nothing about the series of events that took place that day. The complainant had her write up clearly indicate all that happened in a very concise manner. This statement does not. So IMO, it's not the other side to any story because it has no story in it.
R.I.P dear. All men must die.

Dolapo case is not the only one several people have come out to say there experience about the stupid Hosp..the consultant was not available to attend to her, the lab for test was not opened..a lot went wrong.
What about a mad woman that get preg and deliver . On the street without going for antenatal or during our mothers time in the village was there antenatal, so the issue of late registration is scrap..

  64. This is a professionally articulated lie. I expected so much from you Eko hospital knowing you've been in the medical practice for a long time. All Nigerians are not stupid and remember we have well trained medical doctors out there.
Pulmonary embolism does not develop out of the blues. It is not sent by Amadioha or Ogun. There is a preceding event before the embolus is thrown. This woman complained consistently of leg pain and your doctors repeatedly gave her paracetamol. When there was no relief, none of the people you have drafted to write this response could think of DVT (deep vein thrombosis); a diagnosis as simple that a medical student must not even miss. The reason is not far fetched because there was no continuity in care. Different doctors saw her on a daily basis and each felt he was doing well by giving a pregnant/ newly delivered woman paracetamol for persistently worsening leg pain knowing fully well that pregnancy is a hypercoagulable state. Why did the consultant request for a lumbosacral X-ray to rule out fractures in a woman with no prior history of fall? NOBODY IN THE HOSPITAL WAS THINKING!
Mistakes are made in medical practise but do not come here and paint the picture like she just came in, had PE and died. YOU MISMANAGED HER. YOU KILLED HER and had no guts to explain what went wrong to the family. What happens to empathizing with the relatives. What did you learn about breaking bad news?
The family is not interested in making a case so you might not bother doctoring the case note but please employ able hands. Employ those that are passionate about the patients and the practise. Teach them to pick symptoms and signs early and call for help when stranded.
These deaths are totally avoidable. Just one medication would have made a difference in the life of this woman, the newly born child and her family.
We are called to save lives....not take it. Remember, first do no harm!
I hope everyday as you see that baby in your hospital, your conscience would free you from this response you have written carefully....truth or not.

Please be mindful of the kind of statements you make. You just read the statement released, nothing about leg pain was said there. Oversabi and sabongida Ora hinted at some truths, the response was economical in some truths. the leg pain by the deceased should have been a pointer that there"s a risk of DVT which may or may not lead to PE, leg pain in pregnancy is not taken lightly by any DR who know his onions, how much more when ambulation is affected, yes,on the issue that she commenced labour in the elevator, you might avoid that fact as done in your response cos it will buttress your negligence, in all there's an element of culpability on the part of the Hospital and also i stand to be corrected; i thought Lagos state has made it mandatory for autopsy to be carried out for all corpses? please can someone clarify this? Relatives of anyone in any hospital should be vigilant and speak out immediately you perceive any form of neglect and if time permits seek a transfer to another hospital. Till the hospital is involved in a litigation that will open its behind, then it will know that this ostrich defense is risky. RIP DOlapo.....KYLA
